It's almost certainly a bad ballast. You could take the bulb out and try it on the other side, but it's probably not the bulb. If it's sometimes firing and/or flickering, it's not reverse polarity. If you already have a harness, it's getting enough amperage. I'm pretty sure it's the ballast. The cheaper ones just seem to not last forever. The OEM's I've had on my Acuras and Lexuses last forever but the are hundreds of dollars each.
