First, I have no AC, blower only comes on at levels 1 -3, 4&5 don't work and I have a small leak in my radiator. My main issue right now, since its 24˚ outside...
My heat won't come on til the truck has been running for about 15-20 min and MOVING... when I come to a stop it get cold again, then comes back once moving...
I'm thinking THERMOSTAT? Any advice would be greatly appreciated. TRY!

It sounds like you're not getting good flow to your heater core. Thermostat might be hurting, but it might just take 15min to warm up the engine from cold to get the heater flowing. Also, might have air trapped in heater? More likely it needs a flush and has restricted flow.

Blower is a separate issue.

agreed with above. Leak in radiator means air in system. The heater core is at the high point in the truck and that's where the air will go. You need to burp the system Takes just a couple of minutes to do.
Resistor going bad on the blower motor.
