I have the same problem going on in my 97 monte carlo, but mine only starts hard when the engine is warm(after making several stops store ect,) ., rpms fly and it only sometimes dies but runs perfect once started. Im at a loss as well. Only thing i havent done yet is the IAC valve, fuel pressure regulator or a new fuel filter. I hope one of the 3 fix my issue as well. Thing i dont get is that it starts perfect every morning :?

Yeah. Mine also has 200k on it. You are right. Im suprised I dont throw a code either. MAF sensor is new, but i still have original MAP and fuel regulator.so it could be one of those i hope. Hope you figure yours out as well, Good Luck!
