Trans or Rear end "Jolt" on startup

Ive noticed that about half the time when pulling out on to the road, usually when i have to pull out fast for a smaller than i would like opening in traffic, that either the trans or rear end acts like it takes a couple seconds to catch. Though it does happen when im just easing out into traffic too.

The trans had a fluid/filter/gasket change and going to be doing the rear diff fluid/gasket in the next few weeks.

Any ideas of what may be causing that?

Edit: Forgot to add '93 C3500 with 4L80E with 4.10 rear diff gear ratio.

Does it feel like you goy rear ended lightly? If so thats just the very common "clunk" alot of chevys have it and so do other manufacturers. If its not that then the trans may be slipping. Do you notice a spike in rpms when this happens?
